Premier West 5000 Overview

Deductible

Individuals choose to pay a little more, $5000 for the yearly deductible per person, because it lowers the monthly premium. After you've met your annual deductible, Premier-West 5000 shares the cost of medical care with you. We pay 80% of in-network service and you pay just 20%.

Office Visit Copayment

Once you've met your annual deductible you pay just 20% of the office visit. Your Premier-West 5000 plan picks up the rest.

Individual Out-of-Pocket Maximum

You'll be glad to know that your maximum out-of-pocket costs are $8,500 per individual. In fact, you'll receive a lifetime.

Prescription Coverage

Prescription coverage is included in your major medical coverage. That means after you meet your annual deductible you pay only 20% of the cost for prescriptions. Your sign up packet will also list participating pharmacies and mail order information to receive even more discounts and reduce your costs.
