Individuals who want simplicity and flexibility will love this plan. The annual deductible is $5,000 per member with a family cap of $10,000.

Office Visit Copayment

After the annual deductible is met we pay 100%.

Individual Out-of-Pocket Maximum

You’ll be glad to know that your maximum out-of-pocket costs will never exceed the $5,000 per individual and a family cap of $10,000. In fact, you’ll receive a lifetime maximum benefit of $2,000,000.

Preventive Care and Screening Services

Preventive care and screening services have been added to this benefit plan. You will receive well child care, breast cancer screenings, cervical, colorectal and prostate cancer screening along with cholesterol and lipid disorder screenings. First Dollar Coverage Benefit

Deductible is waived for the first $750 of allowed charges each contract year for office visits, chiropractic, accidental injury, and much more. See Outline of Coverage for more details.
