Health Connections Overview

Employees seeking an indemnity plan with a wide variety of deductibles and low coinsurance believe in our Health Connections Plan. You'll appreciate coinsurance as low as zero to ten percent on emergency, urgent care, office visits and more.

Our Health Connections allows you to select the provider of your choice.

Deductible

We offer three separate deductibles within the Health Connections Plan. Choose the low $1000, $2,500 or $5,000. There are no separate deductibles for in or out of network services.

Office Visit Copayment

For those who choose the $1,000 or $2,500 deductible the copayment is only 10 percent. The individual who selects the $5,000 deductible has zero copayment.

Individual Out-of-Pocket Maximum

Because the Health Connections Plan offers three deductible plans within a plan, the out of pocket depends on which deductible you've chosen. For example, a $1,000 deductible means you'll only pay $2,500 ind./$5,000 family out of pocket. The $2,500 deductible has a $3,500 ind./$7,000 family out of pocket and the $5,000 deductible is only $5,000 ind./$10,000 family out of pocket.

Preventive Care and Screening Services

Preventive care and screening services have been added to this benefit plan. You will receive well child care, breast cancer screenings, cervical, colorectal and prostate cancer screening along with cholesterol and lipid disorder screenings. Emergency Care

When you have a medical emergency, cost should be the last thing on your mind. There's no guessing with the Innovations Plan. Depending on the deductible you select your coinsurance on emergency care can be as low as zero or 10 percent.
