How do I know what services are covered?

Refer you're your Outline of Coverage, and your Explanation of Benefits Booklet for information regarding covered services. You can always call Customer Service at 1-800-290-3657 to check on a specific service, to see whether or not the service is covered under your plan.

Who are my providers?

When you enrolled in the policy, your enrollment packet included a provider directory that lists PCPs and other participating providers. You may obtain a more recent provider directory, free of charge, by calling Customer Service at 1-800-290-3657. In addition, you may access the provider directory here.

How do I order an ID Card or a Replacement Card?

How do I submit a claim?

You should submit claims to the Claims Department at:

A claim must be in writing and must include an itemized statement of the health care services that were provided to you, which contains ALL of the following information:

  • Your name.
  • Your birth date.
  • Your member identification number.
  • Diagnosis.
  • Dates of the health care services.
  • Treating provider's name, NWHP identification number (if the provider has one), tax identification number, telephone number and address.
  • Health care services rendered.
  • Charges for the health care services.
  • If applicable, information about the potential liability of any third party for the cost of the health care services, whether or not the third party is insured.
  • If applicable, information about any and all other insurance that is or might be liable for all or a portion of the cost of the health care services, including, but not limited to:
    • Homeowner's, automobile and/or general liability insurance.
    • Medicare, to the extent of what it has paid and/or Medicaid.
    • Health insurance coverage through a spouse.
  • If other insurance has paid any portion of the cost of the health care services, an explanation of payments it made or benefits it provided.
  • An accident report form, if the health care services were required due to an accident.
  • Workers' compensation information, if the illness or injury requiring the health care services occurred in connection with your work.

If the claim is for a prescription, ALL of the following information also must be provided:

  • The prescription number.
  • An itemization for each separate prescription item.
  • The date of purchase.
  • The prescription medication's name, dosage and quantity dispensed.
  • The name of the prescribing provider.
How do I request an individual quote?

Call Strategic Growth at 1-888-500-3355 for a range of premiums for individual quotes. For a medical quote you must fill out a complete application available online, or request one from the strategic growth department. All information must be filled in, and include the HIPAA Authorization form.

How do I request a group plan quote?

Call Strategic Growth at 1-888-500-3355 to request a group plan quote. To receive a census quote, you will need to fill out a Completed Small Group Request Form, and Business Census that is signed and dated by the employer. To receive a medical quote, you will need to fill out a completed Small Group Request Form, Business Census that is signed and dated by the employer, and complete applications signed and dated by each employee. Where do I get my prescription drugs?

Included in your enrollment packet is a list of pharmacies in the New West participating retail pharmacy program. If you need assistance in identifying a participating pharmacy, please contact Customer Services at (800) 290-3657.

Can anyone open a health savings account?

You must be enrolled in a High Deductible Health Plan to obtain an Health Savings Account.

How does one open a health savings account?

You can open a Health Savings Account through a financial institution of your choice.

Can I switch plans?

You can switch plans during your open enrollment period. You may choose to switch plans at any other time of year, but you will be required to fill out applications.

When is the open enrollment period?

Open enrollment occurs on the anniversary date of your policy.
